Case Management

FCA case managers offer comprehensive assessment and in-home services for seniors age 60 and over. We are committed to connecting seniors to available community resources they need to remain safely and independently at home. If you or a senior you know are having difficulty functioning independently at home, our case managers may be able to provide assistance. Case managers will:

  • Provide an in-home comprehensive and confidential assessment evaluating your situation and your needs.
  • Develop a care plan identifying the types of services, the providers and the duration and schedule of service delivery that will meet your needs. This written care plan will be reviewed with you before services are initiated.
  • Make use of all available community resources for which you are eligible, including:
    • Medicaid
    • Food Stamps
    • Home Health Aides
    • Meals-on-Wheels
  • Monitor services to be sure they meet your needs and reassess and modify the care plan when necessary.Offer support for caregivers struggling to care for a loved one.

In-Home Aide Services

Eligibility for in-home services is determined by the case manager during the assessment process.  This includes a financial assessment to determine if these services require a cost share.  There is no fee for case management services. In-home services can include the assistance of a homemaker or a personal care aid, depending on your individual need.

Friendly Visitors Program

Making the lives of homebound seniors better one hour at a time FCA’s Friendly Visitor Program connects trained, screened volunteers with homebound seniors to provide:

  • Socialization
  • Companionship
  • An opportunity to share common interests
  • Good conversation
  • Pursue activities or hobbies

Without the stimulation that social interaction and friendship provide, an isolated senior can experience poor health, a lack of strength and energy, feelings of loneliness and depression. A weekly visit can put a smile on a face and bring joy to the heart. Our Friendly Visitors are warm, kind individuals with an interest in meeting new people and sharing new experiences. Friendly Visiting is rewarding for both the volunteer and the senior. Ancillary Services

This category includes one-time expenditures for “gap-filling” services, items or equipment that cannot be obtained from any other source. These expenditures are authorized by the case manager and the Nassau County Office for the Aging and may also involve a cost-sharing responsibility.

General Information

Case management services are provided, without fee to senior citizens age 60 and over residing in Nassau County in FCA’s assigned catchment area. Counseling services may be available free of charge on a limited short-term basis through the senior wellness program. The case management program is conducted under contract with the Nassau County Office for the Aging (NCOFA) with funds from the Federal Older Americans Act through the New York State Office for the Aging.
