What is the Senior Care and Wellness Project?
The Wellness Project serves seniors age 60 and over, who reside in the designated catchment area of the Case Management Program. The program offers need assessments, supportive services and short term counseling to aid seniors in coping with issues relating to aging. These include: loss and bereavement; coping with changes in medical or physical condition, decreases independence or self-esteem, end of life issues, family problems and other relevant mental health issues which impact the ability of the senior to age safely and successfully in the home. Any senior receiving FCA Case Management Services is eligible and there is no charge for these services.
Who is qualified to use this program?
Any senior currently enrolled in the Case Management Program and residing in the designated catchment area, is eligible to participate in these services at no charge to the client.
How can the Senior Care and Wellness Program help you?
Program staff will assist you with:
- Referrals to appropriate community programs: nutritional counseling, senior programs, case management to assist with accessing concrete services and entitlements, nutritional programs and support groups for clients and caregivers on a variety of relevant topics.
- Providing short-term supportive individual and/or family counseling focusing on the client’s identified issues.
- Information on and referral to relevant resources on medical and mental health issues.
This program is funded by a legislative grant provided by New York State Senator Kemp Hannon.
