Overnight Residential Counselor – Walkabout – Freeport

Walkabout for Young Men and Women A transitional residence in Freeport serving homeless young men and women ages 16-21. The program provides short-term shelter and services for up to 18 months in order to prepare them for independent living.Supervise daily activities of the residents. Manage tasks related to the daily operations of the house and residents. Maintain program paperwork (petty cash, sign outs, timesheets, receipts, logs, etc.)Perform bed checks and morning wake-up when needed.

Program Coordinator – STEERS – Uniondale

Engage state and local partners in the CRTF to ensure the availability of benefits, services, and community-level support to address the criminogenic and stabilization needs of the re-entry population. Schedule, convene, and ensure appropriate representation and participation in bi-monthly task force and bi-weekly case conferencing meetings and develop detailed agendas for each meeting. Notify DCJS of all scheduled CRTF meetings. Educate members of the CRTF about the Task Force’s role as a referral service designed to link returning individuals with necessary services. Serve as the primary liaison between the Department of Corrections and Community Supervision (Parole) to receive appropriate referrals to the CRTF. Coordinate and manage services provided to individuals to ensure their effective re-entry into the community. Serve as a liaison between the CRTF and DCJS which includes completing all required reports by established due dates.

Provide individual and group counseling within the chemical dependency treatment framework and as prescribed by the client's treatment plan.Complete comprehensive psychosocial assessments and develop treatment plans. Coordinate referrals for supportive services including psychiatric, medical and vocational services for primary clients, and when needed intakes. Complete and maintain the case records, reports, treatment plans and all other documentation required by regulatory and referring agencies.
