I have had the pleasure of working for FCA for the last 15 years. I know firsthand that commitment and dedication are needed from all layers of an agency in order for true best practices to occur. FCA not only makes an investment in their clients, but in staff as well. With the flexibility and encouragement of supervisors, many years ago I returned to school to obtain my MSW and continued to grow in the supervisory position that I currently hold. In the Family Ties program we work with families who are vulnerable and often at the lowest points in their lives. Despite the intensity and the emotionally charged day-to-day activities, I feel lucky to be a part of an agency like FCA that not only cares for the clients, but is also committed to helping its staff grow professionally.

Case Planner – Family Ties – Hempstead

Provide casework/outreach services, referrals and crisis intervention for all family members.Conduct and prepare psychosocial intake assessments.Development and implementation of case management goals.Provide counseling, advocacy, referral information and linkage to necessary services.Conduct home and field visits.Compliance with all county, state and federal regulations.

Employee Benefits Administrator – Human Resources – Mineola

Manage and administer the agency's employee benefits, such as medical, dental, life and other voluntary insurance plans offered, as well as the agency pension plan, flexible spending accounts and tax deferred annuity. This includes but is not limited to conducting monthly enrollment meetings, enrolling and terminating employees from benefit plans, processing changes for billing and payroll purposes, managing the online benefits portal and running reports from the portal as necessary, and performing monthly reconciliations between the information on the portal, the provider invoices and the agency's payroll system to identify and correct any discrepancies. Responsible for the completion of employee separation paperwork/processes including providing employees with pertinent benefits information. Manage and administer unemployment claims, ensuring that information provided is accurate and submitted in a timely manner. Assist with management of employee leaves covered by short-term disability, worker's compensation and the FMLA to ensure that the agency meets all legal requirements, including preparing required paperwork and following up with employees or insurance providers as necessary. Maintain required OSHA log.Participate in the annual renewal process for our health and dental insurance and other employee benefits. Research and recommend new employee benefit programs and participate in the development of an employee wellness program.Ensure that data for critical areas of responsibility are maintained in the online benefits portal and HRIS system, or other data bases, so that reliable reports can be generated and analyzed.Assist the Human Resources Generalist with tasks related to recruitment and onboarding of new employees when necessary and as time allows.

Family Engagement Specialist – HCBS & Family Center – Garden City

Support families through personal experience parenting a child with special needs to reduce isolation and enhance their ability to care for their child at home.Facilitate ongoing support groups, workshops and activities; and create networking opportunities.Provide family support services to waitlisted families and Nassau County residents.Provide home visits and case management services to waitlisted families and engage them in services prior to enrollment.

Individualized Care Coordinator (ICC) – HCBS &Family Center – Garden City

ICC Coverage: Provide ICC services from a strength-based and family-driven approach.Provide Intensive In-home and Crisis Response services.Coordinate and monitor all other direct services including sub-contracts.Maintain regular contact with clinicians to aid clients in applying their treatment plan.Monitor and adhere to regulations regarding Flexible Funding, Waiver Budgets, and Medicaid Expenditures.Waitlist: Contact all families at least once a month and make home visits when needed. Engage families in Family Support Services prior to enrollment. Initiate intakes and start-up services prior to ICCs start or return date and when necessary, facilitate the introduction to the family. General: Complete and submit all work by deadlines and comply with all agency and funding regulations.

Meet with families according to their scheduling needs at least 60% of worked time.Increase awareness of and enhance family strengths. Advocate for services and systems that are fair, responsive, and family driven. Assist families to rediscover and reconnect to natural community supports. Promote effective parent-professional system partnerships. Address barriers that may prevent full service participation. Empower families to make informed decisions and advocate on behalf of and in collaboration to promote shared decision making. Prepare and accompany families to meetings to ensure their voice is included in the planning and decision-making at all levels. Conduct groups to offer opportunities for families to connect and reduce isolation. Link to and provide workshops to enhance parenting skills. Support and encourage family participation in community, regional, state, and national activities to develop leadership skills and expand circles of support. Work cohesively with the Individualized Care Coordinator.Collaborate with other family support programs and the OMH Regional Advisor and participate in workgroups that promote family support policy and planning.Complete all documentation by required deadlines and in compliance to agency and funding regulations.

Street Outreach Worker – Street Outreach Project – Wantaugh

Engage street youth through outreach and informal counseling and mentoring. Transport youth to shelter; arrange referrals and follow-up to see if needs have been met.Coordinate services with Nassau Haven and Walkabout staff to help youth exit the streets and secure safe and stable housing.
